OCI SERVICES
In case of submission of OCI applications of minors, both parents need to be present along with the child at the Embassy of India and with all necessary documents to support the application
Documents required to be submitted at the time of OCI application:

Application Form:
To fill up on-line registration form please visit MHA website. Part A has to be filled up online for each applicant of the family and Part B is common to all applicants of the family up to a maximum of four. Separate applications required for any other additional members. After submission of Part A online, a copy of Part A with an online registration number will be printed along with Part B of application form. Details in Part B can be typed or hand written in BLOCK LETTERS. In case the space provided is not sufficient, additional sheets may be used for providing requisite details. No column is to be left unanswered.
Full signature in the box should be provided in case of adults and thumb impression (left hand thumb for male and right hand thumb for female) or signature in case of minor (Under 18 years).
The applicant and the spouse/parents on the basis of whose Indian nationality/Indian origin the OCI card is being applied for, have to be present for a short interview and verification of documents at the time of submission of application form.
1A. If Polish/Lithuanian nationality has been acquired through naturalization, the following additional documents have to be submitted in person at the Embassy
A set of photocopies of requisite documents and each page self-attested by the applicant:

Two photographs with a white background (Click here for photo specification) (one attached to the application and additional one),

Polish/Lithuanian passport copy. If you possess passport of other nationality, then you need to provide Polish/Lithuanian TRC copy,

Renunciation of Nationality Certificate copy

Cancelled Indian Passport along with Embassy/High Commission of India Certificate confirming the surrender of Indian Passport

Naturalization Certificate copy.
1B. If Polish nationality has been acquired by birth, the following documents have to be submitted in person at the Embassy
A set of photocopies of requisite documents and each page self-attested by the applicant:

Two photographs with a white background (Click here for photo specification) (one attached to the application and additional one),

Polish/Lithuanian passport copy. If you possess passport of other nationality, then you need to provide Polish/Lithuanian TRC copy,

Birth Certificate copy.
Conversion of PIO card to OCI card
A set of photocopies of requisite documents and each page self-attested by the applicant:

Two photographs with a white background (Click here for photo specification) (one attached to the application and additional one),
-
Polish/Lithuanian passport copy. If you possess passport of other nationality, then you need to provide Polish/Lithuanian TRC copy,

PIO Card copy.
Acquisition of OCI card as a Spouse
A set of photocopies of requisite documents and each page self-attested by the applicant:

Two photographs with a white background (Click here for photo specification) (one attached to the application and additional one),

Polish/Lithuanian passport copy. If you possess passport of other nationality, then you need to provide Polish/Lithuanian TRC copy,

Indian Spouse passport/OCI card copy,

Apostilled Marriage Certificate.
The processing time of application is around 7-8 weeks. Applicants are required to check their status of the application on MHA website.
