Please note that the Embassy of India in Warsaw does not issue:

Indian Driving Licences, International Driving Permits, Aadhar Cards, PAN cards,

- Bachelorhood certificates or any similar document of such statements,

NOTE
: The Embassy does not issue bachelorhood certificates.

IMPORTANT NOTICE

Embassy of India, Warsaw has received Indian driving licences from Polish Transport Authority.

Indian nationals whose licences were kept by the Polish Transport office, while issuing Polish driving licences, may contact the Embassy for collection/ queries.

2.

Police Clearance Certificate

For Indian Nationals:

- PCC Online form filled online at  
https://embassy.passportindia.gov.in/ and printed.
- One recent passport size photograph,
- Original valid passport and Polish Resident Card or Visa,
- Self-attested photocopy of first and last pages of passport, 
- Self attested photocopy of Polish Resident Card or Visa, 
-  If married and spouse name is not endorsed on the passport, marriage certificate is required.  If divorced, and spouse name is endorsed in the passport, copy of court order is required.

Note: PCC applicants are advised to apply well ahead of time as issue of PCCs would be only be done after due Police Verification.

For Foreign Nationals:

- PCC application form duly filled in and signed,
- One recent passport size photograph,
- Original valid passport and Polish Resident Card or Visa,
- Self-attested photocopy of first and last pages of passport,
- Self attested photocopy of Polish Resident Card or Visa,
- Self-attested photocopy page containing Indian visa(s) on their current/previous passport(s).
 - If the Foreign National had a Indian passport previously, copy of his/her last Indian passport/passport surrender certificate is also required.

3.

Attestation of signature in Power of Attorney
(to be signed before the Consular Officer in person) 

-  Miscellaneous form ( Download the form here) duly filled in and signed. Total 02 recent passport size photographs.

- Original valid Indian passport and its self-attested photocopy of first and last page.

- Original valid Polish Visa/TRC and its self-attested photocopy,

- Original POA document and its photocopy.

- If the POA is related to sale of property, then copy of proof of ownership is also required (highlighting the part which matches to the details in the POA).

- The particulars in the power of attorney should match with the passport of the applicant i.e. Given Name followed by Surname.

- Two witnesses are also required with their original passport and copy of passport. Witness should be Indian Citizens who are living in Poland.

- Copy of one ID of attorney is also required e.g. Passport, Aadhar Card, Voter ID etc.

- Please note 50% space on the bottom of last page should be empty for photo of the applicant and signature/stamps by the Embassy.

Certificate in connection with applying for Polish Driving licence

- Miscellaneous form duly filled in and signed,

- One recent passport size photograph,

-  Original valid passport and Polish/Lithuanian Resident Card or Visa,

-  Self-attested photocopy of first and last pages of passport,

5.

Certificate for students who come for higher studies in Poland

-  Miscellaneous form duly filled in and signed,

-  One recent passport size photograph,

-  Original valid passport and Polish/Lithuanian Resident Card or Visa,

-  Self-attested photocopy of first and last pages of passport, 

- Self attested photocopy of Polish/Lithuanian Resident Card or Visa, 

- Original educational certificates duly apostilled by MEA and
their self-attested copies.

Affidavit from Parent of minor child for issue/re- issue of Passport in India

- Miscellaneous form duly filled in and signed,

- One recent passport size photograph,

- Original valid passport and Polish/Lithuanian Resident Card or Visa,

- Self-attested photocopy of first and last pages of passport, 

- Self attested photocopy of Polish/Lithuanian Resident Card or Visa, 

- Self-attested photocopies of first and last laminated pages of  both
 the parents valid passports,

- Marriage certificate duly apostilled with English translation and
its self attested copy,

- Self attested copy of Passport or Birth certificate of the child,

Application for reissuance of International Driving Permit




Miscellaneous form duly filled in and signed,

- One recent passport size photograph,

- Original valid passport and Polish Resident Card or Visa,

- Self-attested photocopy of first and last pages of passport, 

- Self attested photocopy of Polish Resident Card or Visa, 

- Original existing International Driving Permit and its self attested copy.

- Original Indian Driving Licence and its self-attested photocopy.

Please note : After due processing of the application, Embassy will issue a Receipt to the applicant. Applicant will then need to apply for renewal of IDP through the portal www.parivahan.gov.in and upload all required documents, including the receipt issued by the Embassy.  IDP will be reissued by the licensing authority, i.e. Ministry of Transport and Highways, Government of India and it will be sent directly to the address of the applicant.  All any other query regarding IDP, please visit www.parivahan.gov.in.

Death Certificate 

(i)     For Transportation of body to India, the following documents are required to be submitted to the embassy :-

-       Original Death Certificate issued by the Hospital with English Sworn Translation and photocopy of both.

-       Death certificate issued by the Polish authority (with an apostille by the Polish Foreign office)

-       Embalming certificate and its Sworn Translation in English . 

-       The original passport of the deceased, and it's photocopy

-       Authorization to transport the ashes to India (with Sworn Translation in English) issued by the District Sanitary and Epidemiological Station of the City of Warsaw (at ul. Kochanowskiego 21) .  In order to get this document, you are requested to share the information that  to which city and state the body will be transported. Thereafter, Embassy will issue a document which is required to be submitted to the District Sanitary and Epidemiological Station.
All the above documents are required to be sworn translated to English (except Passport).

On receipt on the above documents, Embassy will issue Death Certificate, Non-Covid Certificate, Transportation Letter.



(ii)    In cases, where the family chooses to cremate the body in Poland and bring the ashes to India, the following documents are required to be submitted to the embassy :-

-  Original Death Certificate issued by the Hospital with English Sworn Translation and photocopy of both.

-       Death certificate issued by the Polish authority (with an apostille by the Polish Foreign office)

- Cremation certificate and its Sworn Translation in English . 

- The original passport of the deceased, and its photocopy

-  Photocopy of the passport of a person responsible to carry the ashes

-   Authorization to transport the ashes to India (with Sworn Translation in English) issued by the District Sanitary and Epidemiological Station of the City of Warsaw (at ul. Kochanowskiego 21) .  In order to get this document, you are requested to share the information that  to which city and state the body will be transported. Thereafter, Embassy will issue a document which is required to be submitted to the District Sanitary and Epidemiological Station.
Please note:  All the above documents are required to be sworn translated to English (except Passport).
